We are able to provide the full range of travel vaccinations and the surgery is a designated Yellow Fever Centre. You do not need to be a registered patient at the Surgery to use this service.
Please complete the Travel Assessment Form in as much detail as you can so that the Practice Nurse can do a comprehensive assessment of your travel health needs and vaccination requirements prior to your appointment. When you have completed the form please hand it in to reception at the Practice or send it by email to mkvp.requests@mkpct.nhs.uk but please ensure that you book a Travel Appointment with the Nurse. Please allow at least 2 working days between providing the completed assessment form to reception and seeing the Nurse for the travel appointment to allow her time to assess your particular needs.
Ideally we need to see you 6 to 8 weeks prior to your planned departure in case a series of vaccinations are required, however travel vaccinations can be administered up to the time of departure and we will try our best to accommodate your needs. Due to a significant demand for nurse appointments we may not always be able to accommodate last minute requests.
During the appointment the Nurse will address travel health, safety awareness as well as administer any necessary vaccinations.
Please note that some vaccinations and anti-malaria tablets are not provided by the NHS and will therefore be subject to fees.
